Radial shock wave device (rESWT)

In 1997, EMS launched the first pneumatic-ballistic system for generating extracorporeal radial shock waves. Based on Newton's principle that movement transfers energy, the company developed the Dolorclast® handpiece for the targeted use of these physical mechanisms of action:

  1. Inside the handpiece, a projectile is accelerated by compressed air and reaches speeds of up to around 90 km/h before hitting a fixed applicator.
  2. Its kinetic energy is converted into radial shock waves, which are transmitted into the tissue in a controlled manner and exert their effect at depths of up to 4 cm.

Focused shock wave device (fESWT)

The piezoceramic principle works by applying a high voltage to 1,000 piezocrystals. This generates 1,000 pressure waves, which form a focused shock wave due to the arrangement of the piezoelectric crystals.

  1. Piezoelectric technology is the only technology that enables direct focusing of shock waves.
  2. Direct focusing enables high energy delivery to the tissue with minimal energy loss. The Dolorclast® Focus handpiece was developed for the constant delivery of high-energy shock waves.

DOLORCLAST® shock wave therapy

Hot peppers and chili peppers contain capsaicin, a substance that stimulates so-called C nerve fibers. These are responsible for pain transmission.

Prolonged activation initially leads to a burning sensation, but then to a temporary "exhaustion" of the nerves - they release less substance P, a neurotransmitter for pain sensation.

How the EMS Dolorclast® shock wave devices work

A shock wave device generates targeted, high-energy shock waves that are introduced into the tissue using a handpiece. This non-invasive technology is used in orthopaedics, physiotherapy, professional sport and veterinary medicine - for example to treat tendon, muscle or bone disorders, to prevent injuries and to support tissue regeneration.

Radial shock wave therapy (rESWT)

Radial shock wave therapy (rESWT) is a non-invasive procedure in which high-energy shock waves have a targeted effect on the tissue. It has the following effects:

  • Pain relief - by inhibiting substance P, which amplifies pain signals.
  • Anti-inflammatory - by reducing neurogenic inflammation and promoting scleraxis expression.
  • Promotion of healing processes - through improved blood circulation and angiogenesis, support for the repair of damaged fibers and regeneration of tendons, muscles, cartilage and bones.

  • A higher energy of the radial shock wave device leads to stronger cavitation in the tissue - the more energy, the greater the pressure and the cavitation effect.
  • This cavitation triggers biological reactions that would otherwise not take place and is crucial for the effectiveness of shock wave therapy.

  • The DolorClast® Focused handpiece has been specially developed to ensure this high energy density even with repeated use and higher frequencies. This allows more tissue depths to be reached and a wider range of pathologies to be treated.

Compared to many other competitor products, the focused shock wave device from EMS leads to significantly better clinical results thanks to its high energy density. High energy per pulse remains crucial for therapeutic success - simply increasing the number of pulses cannot compensate for low energy levels.
